Eligibility
- Benefit eligible employees.
Premiums
- After-tax premiums of $18.00 (deducted via payroll deduction) per month and participation is required for the full calendar year.
Enrollment
- Annual Enrollment period with a Jan. 1st effective date.
- Within 30 days of hire with an effective date of the employee's date of hire.
