Vision Insurance

VSP in-network providers may differ from those in our medical plans. You must go to a VSP provider to have the full vision insurance benefit; otherwise if you out out of network your vision benefit is reduced. If you have a chronic condition such as diabetes or glaucoma, call the benefits department at (414) 288-7305.

Eligibility

All regular, full-time employees (those who work a minimum of 37.5 hours per week on a nine-twelve month annual basis) and full-time temporary employees (who have at least a one year appointment) are eligible. Regular part-time employees can enroll, but must pay the full premium on a payroll deduction basis.

Out-of-network coverage

VSP members get the best value when they see a VSP provider, as this is covered as in-network. Members still have the option to see a non-VSP provider (e.g. Sam's Club, Stein Optical, etc.). The patient is responsible for full payment to the non-VSP provider and has up to six months to submit the itemized receipt and reimbursement form to VSP for a partial reimbursement. Please refer to the Member Benefit Summary or contact VSP's Customer Service at (800) 877-7195 to learn more about the reimbursement maximums.
