Under the supervision of Carolyn Smith, M.D., Executive Director, Marquette University Medical Clinic (MUMC) is pleased to administer prescribed allergy immunotherapy to Marquette Students.

Below are the forms for all new allergy students. Please take time to read over every form before setting up allergy shots.

We do ask that every new student schedule an appointment for an allergy consultation before we will start to administer allergy shots. At this time you may bring your vials and signed doctor's orders with you.

Please be aware there are fees associated with allergy injections.
Full-time undergraduate students or others who have paid the Student Health Fee
- One allergy Injection is $10.00 per visit
- Multiple allergy injections is $20.00 per visit

For all other students (those that have not paid the Student Health Fee)
- One allergy injection is $20.00 per visit
- Multiple allergy injections is $30.00 per visit

Appointments are available Monday thru Friday 8:40am - 3:00pm and must be scheduled in advance. There are no "walk-in" appointments or same day appointments. Injections will not be given if orders from allergist are not current and on file at the time of your appointment and you will need to reschedule.

We do have a 10 minute late policy. If you arrive more than 10 minutes late you will need to reschedule your appointment.
