- Marquette University logo, as indicated in the illustration
- Personal information, which includes name, highest degree earned (optional) and title
- Name of the college and/or department/office/division on one or two lines
- Address information includes:
Building name/designation, room number

A home telephone number may be included if used regularly for university business. Home addresses may not be included.
