Federal Veterans Benefits — How to Apply Every term, all students who are eligible to receive federal veterans educational benefits (new and continuing) must complete the following university form: Application for Certification of VA Educational Benefits (PDF) First-Time Use of Benefits If you have never used your Veterans Affairs (VA) educational benefits you will need to provide documentation of your eligibility to receive benefits. Required Documents: Chapter 30, Active Duty GI Bill – Copy of Eligibility letter from the VA or a copy of your DD 214, Certificate of Discharge. Chapter 1606, Reserve/National Guard GI Bill – DD 2384, Notice of Basic Eligibility. Chapter 1607, TBD (To Be Determined). Chapter 35, Survivors’ and Dependents’ Benefits – Copy of Eligibility letter or complete VA 22-5490, Application for Survivors’ and Dependents’ Educational Assistance. Chapter 31 – Documentation will be provided by VA Voc- Rehab Office. If you are eligible for an Educational Bonus/Kicker (primarily Chapter 30/1606 recipients), you will also need to bring in the appropriate documentation in order to receive the benefit, e.g., DD 1131, Cash Collection Voucher.
