As a student at Marquette University, you are eligible to purchase the university-sponsored Student Injury and Sickness Insurance Plan underwritten by United HealthCare, and administered by UnitedHealthcare StudentResources. This plan has been carefully designed to complement care at the Student Health Service, and can serve as a supplement to family plans, or as an option for students to use as their sole coverage.
We recommend that you check with your family’s health insurance carrier to see what benefits are available to you in the Milwaukee metropolitan area. If your coverage is limited and/or you have a high deductible and coinsurance, you may want to consider the Student Injury and Sickness Insurance Plan.
For a full description of coverage (including eligibility, costs, benefits, exclusions, any reductions and limitations, and the terms under which the coverage may remain in force), please refer to the 2008-09 Policy Brochure, below; or, visit UnitedHealthcare's StudentResources website at www.uhcsr.com and select "Find My School's Plan."
To enroll in the Student Injury and Sickness Insurance Plan, complete and return the 2008-09 Enrollment Form (or the Continuation Form, if you were enrolled in Marquette's plan during the 2007-08 school year), located below. You can also enroll online by signing on to the carrier’s website and selecting "Enroll online now." For further information not provided at the carrier's website, please contact UnitedHealthcare StudentResources at
